Representing Community

The Community Health Programs at Children’s Institute support care coordination, system navigation, and a full range of support for health services to improve day-to-day living and wellbeing. These programs include Enhanced Care Management, United Mental Health Promoters, Community Public Health Teams and Children Exposed to Violence – REACH Team, and consist of Community Health Workers, Care Coordinators, Care Managers, Health Promoters, and therapists who work tirelessly in ensuring individuals and families obtain the support they need.

In appreciation of their hard work, commitment and dedication, on May 29th the Community Health Division had their first retreat. This retreat consisted of self-care activities, group activities to promote interaction amongst teams, and training. Managers presented on whole person care and how it includes a care team and cross sector engagement. They discussed how we all work together to address complex issues and achieve shared goals.

After the presentation, everyone engaged in a community mural project as a reminder that we are all part of a larger team and together we can create something amazing. This mural is a representation of the collaboration and connection of the Community Health Programs at Children’s Institute and the impact they are making in their communities.
